If you say something whilst joking with friends and say "macka (means sandwich in swedish) instead of Makkah, are you considered to be kafir? and do you have to repeat all your salahs and fasts?

The statement you mention is certainly one that should never be uttered by a believer and one should do Tawbah for that and in the future be very careful in regards to all that you say. If you have doubt about the suitability of saying something, then it is wise simply not to say it, as the consequences of uttering words of disbelief are very severe. Having said this, the statement you said does not take you out of Islam.
